Peer Specialist:

 

The role of the Peer Specialist is to support the residents participating in the CCD CARE (Community Advocacy & Recovery Empowerment) Program. This is a role in which the incumbent must develop a rapport with each resident and serve as their advocate, assisting throughout the provisions of their recovery plan, and monitoring their progress throughout their participation in the program.  S/he will provide case management services, discharge planning, and care coordination for clients. S/he will maintain an independent caseload and will ensure proper referral of clients to all appropriate services and resources available, including, but not limited to, medical care, health and social services benefits, education and employment, recovery and prevention services, and aftercare supports. The Peer Specialist is responsible for monitoring referrals for appropriateness, conducting phone screenings with potential residents, providing intake services to residents, conducting ongoing needs assessments to ensure the development of individualized recovery plans, and developing a post-discharge plan with each resident that addresses housing, social supports, vocational training, and employment.  

 

S/he must identify as a peer and have personal, lived experience in their own recovery from an alcohol or drug addiction. The preferred candidate will have a certification through the Delaware Certification Board as a Certified Peer Recovery Specialist. Any individual who is not certified must obtain certification within one year of employment. Minimum 1 year experience in related field. Valid driver’s license required.

Outreach Specialist:

 

The role of the Outreach Specialist (OS) is to support the CCD CORE (Community Outreach, Response & Engagement) Program. Working in the community, the OS will connect adult individuals with substance use disorders and co-occurring mental health conditions with resources to meet their needs. The incumbent in this position will provide care coordination, resource linkages, and guidance to help individuals navigate social services benefits, health insurance, and other community resources such as harm reduction and treatment services. The OS must develop a positive rapport and serve as an advocate, assisting individuals with their goals in their wellness plan as well as monitoring their progress.   

 

The OS must identify as a peer and have personal, lived experience in their own recovery from an alcohol or drug addiction. The preferred candidate will have a certification through the Delaware Certification Board as a Certified Peer Recovery Specialist. Any individual who is not certified must obtain certification within one year of employment. Minimum 1 year experience in related field. Valid driver’s license required, with no more than 2 points.
